The Chiropractic AI Visibility Index.

The Chiropractic AI Visibility Index records the practices Google AI Mode named in 40 US cities, then reads 460 chiropractic practice homepages to see what those sites carry.

Answers captured September 8, 2026, and homepages read September 12, 2026 by KKV Labs LLC. $1,000 a month, 3-month minimum as it takes time to show results, named by AI in 90 days or every dollar back.

The finding, in three sentences

Google AI Mode named 192 chiropractic practices across 40 cities. That is 4.8 names per city on average, never fewer than 2 and never more than 7.

We then opened 602 chiropractic homepages and read 465 of them. 460 of those showed chiropractic work on the page and are counted here. 105 are practices the answer had named. The other 355 are everyone else we could read that day.

The separations are about what a homepage writes down, not what it runs. Every comparison on this page is a same day reading rather than a cause, and none of it is a statement about anybody's care.

The findings

Each one is a check you can run on your own site in a minute. Every share is read from the 460 homepages we counted on September 12, 2026.

40 of 40

Every city already has an answer

Google AI Mode came back with practice names in every one of the 40 cities we asked, 192 names in all. The list is short every time: about 4.8 names per city, never more than 7. The job in this market is joining a list that already exists.

The named practices write more and list more

A blog or articles section is linked from 67% of the named homepages and 44% of everyone else. Knee, shoulder or joint pain sits on 57% against 40%. The named homepages run a median of 1,135 words to 821. Nothing in the technology stack comes close to those distances.

26% and 25%

Credentials sit on the same number in both halves

Years in practice is stated on 26% of the named homepages and 25% of everyone else. Board certified is 3% against 3%, and DC after a name 24% against 22%. Naming the chiropractic school reads the other way round, 7% against 14%. On this day, credential polish was not what told the two groups apart.

The markup that names this business is wide open

84% of the homepages we counted carry structured data of some kind and 58% say in it that the business is a practice or a local business, but only 2% use the Chiropractor type that exists for exactly this business. The named practices sit on 53% of that middle line against 60%, so this is open ground rather than catching up. It is a one line change on most sites.

We asked Google AI Mode the same question in every city: who is the best chiropractor here. It came back with names in all 40 of them, and the list was short every single time.

A short list is the finding. The practices on it hold the whole answer for that question on that day, whatever anyone else spends on ads.

A tool that only appears after a button click is not counted, so this is the floor rather than what the trade runs. Call tracking sits in the marketing stack below, so one script is never published as two shares.

This is the sharpest difference between this market and the other indexes we have run. A live chat, text or answering widget sits on 1% of the 460 homepages we counted, against roughly a quarter of personal injury firms. What this market does use is a plain booking route, on 60% of homepages, and that is one of only two technology rows on this page that separate the two groups: 68% against 58%.

The marketing stack

What the market pays to install.

Tracking, advertising and call tracking scripts, read off the homepage. A tag is the cheapest thing a practice can add to a site, and it is usually the first thing that gets added.

How to check our work.

Citation

Use any figure on this page. We ask for one thing: name the source and link to this page, so a reader can see the date and the method.

The city sample
40 US cities across 23 states. We found 580 chiropractic websites in them and screened 458. This is not a random sample of all US chiropractic practices: it is the markets we were looking at, which lean toward small and mid size cities. A practice with offices in two of these cities is listed once, in the city we found it in first.
The AI answers
One question per city, asked in Google AI Mode from a fresh logged out session, with the practices recorded in the order they were given. 40 cities were recorded September 8, 2026. Answers change. This is a dated snapshot, not a ranking and not an endorsement of any practice on it.
The technology crawl
On September 12, 2026 we opened 602 chiropractic homepages and read 465 of them. 92 answered and turned the crawler away, and 45 did not hand back a page we could read. Detection reads the homepage HTML for known scripts, links and markup, so a practice can use a tool without it showing here. Every share is a floor.
The relevance gate
A site is counted only when its own text or one of its own page links shows it is a chiropractic practice. That dropped 5 of the 465 sites we read, which leaves the 460 every share on this page is measured against. The gate is deliberately literal: what the page says is what is measured, so a clinic that does this work and never writes a chiropractic word reads as out of scope rather than as a low score.
Named versus unnamed
The named set is the 105 crawled sites Google AI Mode listed in their own city answer. The other 355 are everyone else. Comparing the two shows what the named group happened to have that day. It does not show that any of it is why they were named, and we do not claim that anywhere.
How a city was coloured
The colour is the length of the answer: GREEN is three or more practices named, YELLOW is two and RED is one. 39 cities came back GREEN, 1 YELLOW and 0 RED. No city was coloured by hand. With almost every city in the same bucket the label sorts very little here, so read the practice list rather than the label.
What we are not claiming
Nothing here says a practice with better markup will be named, and nothing here compares anybody's care. A practice named by an answer engine is not being called a better chiropractor than one that is not on the list. This page says what this market looked like on two dates in September 2026, and which of the cheap signals are still open.

Caveats

What this data cannot tell you.

Read these before you quote a number from this page. They are the honest limits of the run, and we would rather print them than have somebody find them later.

A same day comparison, not a cause
The named practices were named for many reasons this crawl cannot see, including reviews, how long they have been open, local press and who links to them. Nothing on this page is a ranking factor, and nothing on it is about clinical care.
Almost every city is in the same bucket, so the label says little
39 of the 40 cities came back with three or more practices named and 1 came back with 2, so the colour does not sort this market the way it sorts one with quiet cities in it. The useful column is the practice list and the count beside it.
58 named practices have no website we could resolve
Google AI Mode gave those practice names with no link, and no first-party record we hold maps the name to a site. They are counted in the city table, which is built from names, and left out of the named versus unnamed comparison, which is built from websites. We resolved 134 sites and read 105 of them, so that comparison rests on 55% of the practices the answer named. That is the lowest of our indexes so far, and the reason is in the next row.
8 websites were thrown out before we published
Chiropractic practice names are short and shared, so a guessed hostname lands on the wrong business more often here than in any market we have measured. Every match has to pass three tests: a distinctive word of the practice name in the title or the markup, address shaped evidence for the city it was named in, and at least five chiropractic words on the page. 8 rows failed and lost their website, including a newspaper, a trade media company, a software startup, a parked domain and three real practices in the wrong city or the wrong state. One weak row was kept with the evidence written down.
10 practice names appear in more than one city
Names like Core Chiropractic and Smith Chiropractic come back in two different cities, and those are two different businesses. So the page reports 192 names across 40 answers rather than a count of distinct practices, which would quietly merge them.
92 sites are behind a bot wall, and they are not a random group
92 of the 602 sites we opened, 15%, answered and refused the crawler. A practice spending heavily on its marketing site is more likely to run that kind of front door, and 16 of the 134 named websites sit in that bucket. Every share here is measured on the sites that let us read them.
The per-city site counts are not comparable with each other
The candidate list was widened on the day of the crawl, and the pass reached 21 of the 40 cities before it stopped. Those cities now carry two or three times the candidates of the other 19. So a city's site count measures how much searching that city got, not how many chiropractors practise there. The thinnest three are Midland, TX, Beaumont, TX, Lubbock, TX. The city answers are unaffected: they are captured from the AI, not from the candidate list.
Homepage only, so every share is a floor
A certification on an About page, a price on a new patient page or a booking tool behind a button is not counted. Detection is by known scripts, links and text, so a new or self built tool reads as nothing at all.
A technique is counted only when the homepage names it
A practice that uses a technique every day and does not write it on the homepage is counted as naming none, and the median practice in this index names none. The technique rows are a reading of what the page says, not of what happens in the room.
The care plan row counts words, not a product
"Your care plan starts with you" and "12 visit package" both match the care plan check, which is why it is the softest row in the comparison even though it is one of the widest. Quote the blog row and the service range rows ahead of it.
No speed numbers, on purpose
The crawl ran from India. Response times were recorded and are not reported here, because from that distance they measure the length of the wire rather than the speed of the site.
Small bases on some rows
The named side is 105 sites, so several rows in the comparison rest on a handful of practices, and the site counts are printed next to every share. Quote the counts. Never say twice as likely.
The state table runs on small bases too
23 states had at least 8 sites counted. Most of them are a single index city, so a state figure there is really a city figure.
A partial robots.txt rule is not an AI block
Most of the robots.txt files we read disallow some path for every crawler, which is what the rule a website builder ships looks like. We did not keep the rule text, so we report only the outright blocks and leave the rest alone.
The crawl is later than the capture
The answers were captured September 8, 2026 and the homepages were read on September 12, 2026. A site can change in four days, so read the answer column against its own date and the technology shares against theirs.

What people ask about this data.

What is the Chiropractic AI Visibility Index?
A first-party count of chiropractic AI visibility: which practices Google AI Mode named in 40 US cities, and what the crawler could read on 460 chiropractic practice homepages on September 12, 2026. Nothing is bought, licensed or estimated.
Is there a city where AI has no answer yet?
Not in this index. Google AI Mode named at least 2 practices in every one of the 40 cities, and 39 of them came back with three or more. The most common answer is 5 names, in 21 cities. So the work here is joining a list rather than starting one.
What did separate the practices AI named?
What the homepage writes down. A blog or articles section is linked from 67% of the named sites and 44% of everyone else. Knee, shoulder or joint pain sits on 57% against 40%, and Pediatric or children's care on 40% against 24%. The named homepages run a median of 1,135 words to 821 and list 12 services to 10. That is a comparison on one day, not a cause.
Do credentials help a chiropractor get named by AI?
We cannot see that in this data. Years in practice is stated on 26% of the 105 named practices and 25% of the other 355. Board certified is 3% against 3%. Naming the chiropractic school reads the other way round, 7% against 14%. Credentials are worth printing for the patient reading the page. On this day, in this market, they were not what told the two groups apart.
Which practice and booking software do chiropractors run?
On the 460 homepages we counted on September 12, 2026, 23% showed a practice, booking or patient engagement tool we could detect. The most common were Jane at 10%, ChiroTouch at 5%, SKED at 2%. 49% of the sites run on WordPress. A tool behind a button is not counted, so these are floors.
Does an online booking link matter for a chiropractor?
It is one of only two technology lines in this index that separate the two groups. A booking route on the homepage sits on 68% of the named practices and 58% of everyone else, and a printed review count on 17% against 7%. Both are a morning's work on most sites. It is still a same day comparison, not a cause.
Do chiropractic websites block AI crawlers?
Almost none of them do. Of the 432 sites that served us a robots.txt, the most blocked AI crawler is named in a blocking rule on 3% of them. A sitemap is reachable on 95%. Access is not what decides who gets named here.
Can I cite these numbers?

Is a practice named in the answer a better chiropractor?
No, and we do not say that anywhere. The lists are a dated snapshot of what one AI answer said on one day. They are not rankings, not endorsements, and not a judgement of anybody's clinical work.
How often is this updated?
We re-run the city answers and the crawl as we work these markets. Every figure on this page is computed from the data files at build time, so the report updates the moment the files do, and every AI answer carries the date it was captured.
