Free med spa playbook: the money side

How to get Botox patients without Groupon or RealSelf

Here is how to get Botox patients without Groupon or RealSelf: count what a discounted first visit really costs you, then put that effort into the free listings AI answers read. RealSelf showed up in none of the 21 med spa AI answers we captured on 15 September 2026. A Google profile showed up in 21 of them and Yelp in 9, and both are free.

What you need

  • 30 minutes a week for 90 days
  • No money. This plan ends with you spending less
  • Last year's deal site and directory invoices, and your booking software login
  • One page on your own site where you can publish a price

Time: about 30 minutes a week, for 90 days. Cost: nothing. Every step below is free to do yourself.

Go straight to the 10 steps

Start here

What a voucher really costs.

Every number here is counted from our own files when this page is built, or linked to where it came from.

Start with your own sums, because nobody else's will do. Zenoti's 2026 benchmark data for 2025 puts the middle med spa ticket at $216, and one ticket in ten at $484 or more. AmSpa's industry survey, quoted in a December 2025 release, puts the average spend at $527 a visit. Those two measure different things, so we never split the difference. Take either one, cut it by the voucher discount, then cut it again by what the deal site keeps. Groupon's merchant FAQ says there is no fixed percentage and that every rate is agreed before launch, so the only true figure for your clinic is on your own invoice.

Now the platform side. We read 1,352 med spa home pages on 10 September 2026. RealSelf is linked from 1% of them and from 0% of the clinics AI named. In the 21 AI answers we captured on 15 September 2026, RealSelf appeared in none, while a Google profile appeared in 21 and Yelp in 9. We found no published rebooking rate for deal site patients, so we will not invent one. What AmSpa wrote in 2018 is that up to 70% of patients at an average med spa are repeat clients, that a new client costs over 5 times as much as keeping one, and that existing customers spend up to 67% more.

The plan

Ten money jobs for a med spa.

One job a week for ten weeks. Each one is about where a Botox patient comes from and what she costs you, and how to tell it worked.

  1. 01

Price one voucher visit on paper before you sell another one

Write the voucher price at the top of a blank sheet. Take off the deal site's cut from your own invoice, not from a blog. Then take off the product, the room and the injector hour, and compare what is left with the $216 middle ticket in Zenoti's data and the $527 average visit in AmSpa's survey. While you are at it, check your state's rule: AmSpa's founder, a lawyer, wrote that in most states a deal site taking a cut of a medical treatment is a textbook case of fee-splitting.

How to check it workedOne sheet shows the voucher price against the $216 middle ticket.

  2. 02

Count how many voucher patients came back at full price last year

Pull last year's voucher list out of your booking software. Mark every patient who booked another treatment and paid full price, then divide. That is your real number. We looked for a published rebooking rate for deal site patients in this trade and found none, so yours is the only one there is.

How to check it workedYou can say the rebooking number out loud with a date on it.

  3. 03

Look at the discount from a full-price patient's chair

Your regular patient gets the same treatment, in the same room, from the same injector. Then she sees the same treatment at the voucher price online. AmSpa warned in 2018 that clinics that run mostly on deal sites tend to get bad reputations. Decide what you will say when a loyal patient asks why a stranger paid less, and write it down before you need it.

How to check it workedYou have a written answer for the regular who asks about the deal price.

  4. 04

Check what a directory listing is really buying you

Pull every invoice from RealSelf or any other directory for the last 12 months and add them up. Next to the total, write how many bookings your call log or booking software says came from it. RealSelf is linked from 0% of the clinic home pages AI named in our crawl, and it appeared in none of the 21 AI answers we captured. In our data the answers lean on your Google profile and your Yelp page, not on it.

How to check it workedYou know the yearly figure you pay and the bookings it produced.

  5. 05

Replace the voucher with a treatment package or a membership

A voucher sells one visit to a stranger. A package or a membership sells several visits to the same person. Price it so the patient has a reason to stay and you have money that arrives each month. Keep the wording to the visit schedule and the price, and leave every claim about results off it.

How to check it workedOne treatment package or membership is live with a monthly price.

  6. 06

Turn on Alle, the one rewards program that runs the right way in our data

Alle shows on 9% of med spa home pages. Among the clinics AI named it runs at 11%, against 8% for the rest. The other two rewards programs we found are flat: RepeatMD at 4% against 4%, Aspire at 2% against 2%. The payment plan Cherry runs backwards, 9% against 12%. It is a small gap on a same day count, not proof of cause, so treat Alle as the cheap one to turn on rather than as a lever.

How to check it workedAlle is enrolled, live, and linked from your own site.

  7. 07

Ask every caller how she found you and log it by treatment

Put a note on the front desk phone with one question on it. Write the answer next to the treatment she booked, not only next to her name. After 60 days you will see which source sends Botox and which sends laser. Only 13% of the med spa home pages we read carry a Google Ads conversion tag we could detect and 20% a Meta pixel, so a paper log is often the only record a clinic has.

How to check it worked60 days of call logs show a source and a treatment on every line.

  8. 08

Publish a starting price for your three most shopped treatments

Only 39.9% of the 1,352 med spa sites we read show a price on the homepage or a linked pricing page. Pick your three most asked about treatments and give each a starting price on your own site. Be clear about why: a price does not change whether AI names you, since named clinics show one 40.5% of the time against 39.7% for the rest. You are doing this for the patient who is calling three clinics, not for the machine.

How to check it worked3 starting treatment prices are live in text on your site.

  9. 09

Book the next visit before she gets out of the chair

A deal site patient is a stranger who has now met you. Ask for the next appointment while she is still in the room and put it in the book there and then. Write a short script for this and use the same one every time. Repeat patients are what AmSpa's figure of up to 70% is made of.

How to check it workedYour calendar shows a next treatment booking on most voucher visits.

  10. 10

Pick the date you stop the deal site spend and hold it

Choose a date 90 days out and write it on the wall. Until then, fix your Google profile and Yelp page and watch your own bookings. On the day, cancel or cut the spend, then compare the next 30 days of Botox bookings with the same 30 days last year. Bookings through your own site and your Google profile cost you nothing per click.

How to check it workedThe spend stopped on the date and you have two 30 day booking counts to compare.

Our own numbers on this page: 245 US cities asked "best med spa in [city]" in Google AI Mode on 8 September 2026; 1,352 med spa home pages read by machine on 10 September 2026, out of 1,531 we tried; and a separate study of 21 city answers captured on 15 September 2026 that recorded every link the answer pointed at. Every figure above is counted from those files when this page is built. Anything from outside our own work is named where it is used, and linked where the page allows it. Published by KKV Labs LLC.

Questions

What patients ask about Botox.

Three questions a Botox patient asks before she books. Answer each one on your own site in plain words, and leave every promise about results out.

How much does Botox cost per unit?
Prices differ by clinic and by city, and we do not repeat the per unit ranges passed around online because we could not trace them to a source. What a clinic pays is a separate number: decodahealth.com reported in August 2026 that AbbVie's published wholesale list cost is $656 for a 100 unit vial of Botox Cosmetic, which is $6.56 a unit before any discount. Publish your own starting price so the patient and the answer both have a real figure.
Is it safe to get Botox at a med spa?
Do not answer this with a promise. Patients may ask because in April 2024 the CDC warned about counterfeit or mishandled Botox-type injections given by unlicensed or untrained people, or in places such as homes or spas. On your site, say who injects, what license they hold and who supervises them, and check your state's rule for the wording.
Do you need a consultation before Botox?
The rule depends on your state and your medical director, so we do not state one here. What you can do is write your own clinic's policy on your site in plain words: whether a first visit starts with a consultation, who does it, and how long it takes. An AI answer can then repeat your policy instead of guessing.
